  • A Class 1 medical certificate.
  • A Private, Commercial or Airline Transport Pilot License, on which to endorse the rating.
  • A properly completed prescribed application Form.
  • Personal logbooks or other acceptable flight records substantiating the flight experience shown on the prescribed application form.
